Helpful Forms

Students are able to complete a number of processes using the Student Self-Service feature available in the SIS student information system. This includes requesting health insurance waivers and viewing your transcripts.

  • Request Information - to request a program brochure or to have a general brochure be sent to your home.
  • Non-Degree Course Registration Form - register for a course.
  • Degree Application - (pdf format)
  • Certificate Application - (pdf format)
  • Certificate Petition Forms - To be completed by all Undergraduate certificate students when they complete their certificate program (pdf format).
  • Graduate Certificate Clearance Form
  • Undergraduate Student Readmission Form - Students who have previously applied into a program at UMass Lowell and have been inactive (haven't registered for courses) for more than four semesters will need to apply for readmission before they can register for courses. Note, if you have only been inactive for a couple of semesters, call or email the Registrar's Office at 978-934-2550 or Registrar@uml.edu and ask them to re-activate your student account.

    Graduate Readmission Process: Graduate students must register for each Fall and Spring semester until completion of their program. Students who need to take a semester off must register for CONT.6010 (Continued Matriculation) which is limited to two semesters for students pursuing a Master's and three semesters for students pursuing a doctoral degree. Beyond this, students must contact Graduate Admissions and reapply to their program.
  • Change of Program Forms
  • Course Status Form - to request "pass/no credit" or "audit" status (pdf format)
  • Student Information Change Form - to make changes to how name, telephone, and/or billing, local, permanent, or email address appear within the SIS student record system (pdf format)
  • Health Insurance Waiver - (see Student Self-Service in SIS)
    Log into your SIS account and select "Waive/Enroll Health Insurance Form" Required for those students who are enrolled in at least 9 semester hours or more in a given semester.
